2023 School Sports Physical Event for $25 on August 2, 2023 from 3pm - 7pm Amberwell Primary Care 300 Utah Street Hiawatha, KS 66434 This one-time event makes getting high school and middle school school sports physicals easy and convenient. The cost is $25 per student at the time of the event. If this date does not work for you or your family, sports physicals can also be scheduled with an Amberwell primary care provider at a time more convenient for you by calling 785-742-2161 Reminders • Athlete must wear athletic attire. No boots, flip flops, or jeans. • KSHSAA Physical Form must be filled out by PARENT and ATHLETE prior to arriving. • Parent must be present with athletes 8th grade or younger. • Vaccination history and well child visit check is included.
