Getting Your Child Ready For Kindergarten

IMPORTANT NOTICE FOR YOUR NEW KINDERGARTEN STUDENT!!

 Prior to entering kindergarten in a Kansas public school, the following immunization schedule should be completed (all of the below immunizations are required if an (*) is indicated).

*DTaP:  5 doses required; 6 months between dose 3 and 4.  Four doses permissible if 4th dose is after 4th birthday.

*IPV:  4 doses 4 weeks between each dose, OR 3 doses if one dose given on or after 4th birthday

*MMR:  first dose must be given on or after first birthday

Hep A:  Not yet required, but recommended

*Varicella:  first dose on or after first birthday, booster required for entrance into kindergarten.

Your student will also need:  Kansas Physical/Health Assessment that is dated no earlier than ONE YEAR from start date of the date they will enter kindergarten.

IF YOUR CHILD WILL NEED MEDICATION AT SCHOOL, PLEASE VISIT OUR WEBSITE FOR GUIDELINES AND PERMISSION FORMS.

According to Kansas immunization laws, USD 415 does impose an exclusionary date on children who do not have or present a completed immunization log/current physical to the school.  It is imperative that this material is turned in at enrollment to assure your child will be able to attend school.

Please bring to enrollment:

     Completed Immunization card and/or KCI form (filled out by nurse or 

     physician)

     Copy of Current Kansas Well-Child Check

     Health History Form

     Certified Copy of Birth Certificate
