Immunization Clinic for 6th Graders

*TO THE PARENTS OF 11 YR OLD STUDENTS, Birth dates from 9/1/97-8/31/98*

*(6th GRADERS)*

In order to assist families to comply with the new Immunization required for 11 yr olds
(6th grade), the Brown County Health Dept. will hold an Immunization clinic for the Tdap
immunization, on Tuesday, June 30, 2009 from 9:00 a.m. until noon and 1:30 p.m. until
4:30 p.m. at the Brown County Health Dept., 907 S 2nd street.

The cost for this injection will be $14.00.

PLEASE BRING YOUR CHILD’S IMMUNIZATION RECORD AND PROOF OF MEDICAID (IF APPLICABLE)
