Getting Ready For Kindergarten 2009-2010

The Nurses at USD 415 want to welcome your new student! There
are so many new things they will be learning this upcoming year! As
parents, it is hard to keep track of all of the required forms and
documents that are needed from the school. Please use this sheet as
a general check list to ensure that when you enroll your student
they are ready to start their year with all the required
paperwork.
If at any time, you should have a question about what is required
for your child, please do not hesitate to contact the school nurse,
and or building administrator. Please read below to learn about the
state health requirements for your new student.
Health Physical or
well child check:
• Is your child’s physical dated by the doctor no
earlier than one year from the start year of the 2009-2010 school
year: 8-19-2009
• Has your child’s physical or KanBe Healthy screen been
completed by a Kansas doctor/or health care professional licensed
to perform health screens?
Immunizations:
• Has your child had all of his/her immunizations from
birth to five years?
• Do you have records of all of his/her immunizations?
• Have you read through the new immunization requirements and
checked with your family physician to determine if any vaccines are
needed to keep your child up-to-date?
• Has your child had chicken pox? If so, do you have a note signed
by your physician stating verification of the disease. This can
also be done on the KCI form (please see enclosed)
• If your child has not had the chicken pox, has he/she been
immunized with 2 doses of Varicella?
• KCI Form: has your family physician filled out and signed the
Kansas Certificate of Immunization form to be turned into the
school nurse?
Dietary
Restrictions:
• Does your child have a food allergy? If YES, has your
family physician filled out a form for the school stating this? All
local doctors have been provided with a state required form from
the school-this form must be used for the state in place of a note
on a doctor’s script pad. Talk with your family physician if your
child has an allergy or intolerance to food.
Medication:
• Will your student need to take meds while they are
here at school?
• If your child will have medication administered at school,
over-the-counter or prescription, a medication permission form must
be signed by both the physician and the parent—all medications must
come in original container and be labeled properly with student’s
name. Please visit "nurse's station" to view medication
administration guidelines at school.
Health Concerns:
• Does your child have health concerns that the school
should be aware of?
• If your child has health concerns, do not forget to let your
school nurse know so that a care plan can be created if needed, and
so that staff in direct care of your student can be aware.
Remember, it is the responsibility of the parent/guardian to let
the school know of any changes in medication and/or medical
condition your student may have.
Forms
• Do you have all the above forms ready to turn in at
enrollment?
• It is Kansas state law that immunizations and well child checks
must be up-to-date and copies furnished to the school in order to
remain in a Kansas public school. Please have forms properly filled
out and signed by the appropriate people when you come to enroll
your student.

If your child will be taking medication at school, please print a properly fill-out a "Medication Permission Form" off of this web-site under "Nurse's Station".
Please print and complete the "Student Health History Form" and return at Kindergarten Round-up or Enrollment for 2009-2010 school year.
